Before you begin, please check out our curated Music Library if you’d like to add background music to your reel. Once you’ve selected a song, return to this page to fill out the form. 

Your answers should save automatically as you complete the form, and you can use the Back button at any time to return to a previous page and review or update a response.

At the end of the form, you’ll be able to upload your athlete’s photo and video clips before submitting.

After you submit the form, you’ll receive an email with a copy of your answers and uploaded files for your records. If you need to make any changes or provide additional information after submitting, simply reply to your confirmation email as soon as possible so we can include your updates and keep your project on track for a quick turnaround.
